I turned out, and I was asked if I would be willing to serve on the as a community member on the Hawaii Dental Board, and I said I was very happy to. I am the executive director for Pu'ulau La Pa'au, the Hawaii program for health care professionals, and it is a monitoring agency that monitors health care providers who require licensing through the DCCA. When I sat on the Hawaii medical board, a number of individuals who are our program participants were up for discussion, and I always recuse myself.

    Thank you for being here. Thank you for serving. Question for me. This session, we have a, we have a shortage in psychologists. There's a bill that opens Hawaii up to a compact.

    Our stance has always been against compacts because we want locals to have these jobs. I know there's reasons why it's hard for that to happen. But anybody that I vote to serve in a leadership position, especially with this, I have to know where they stand on that, if you could explain.

    For any qualified potential psychologist, we'd be looking, as a board, to support that. I do know that there's increases in the training programs, because I'm involved with Chaminade. HPU started a program. And so, there is increases in the trainee level that's going to hopefully be filling some of those gaps. It's just, it's gonna take a while, and so, we're looking to, to, at least on my professional side, to fulfill some of those needs that way.

    Sherrie, you're going second on my questioning, so I'm I'm sure you already heard the first line, but I if I can get your take on the compacts when it comes to psychologists in Hawaii.

    So what I think one of my concerns about the compact right now, I think has to do with, cultural sensitivity and awareness of our community. I do feel that Hawaii's cultural milieu is very unique compared to other states in the compact. And so I guess that's one of my main concerns. So I think it's just something that I would need to give a little bit more thought on. I do, you know, believe that there's a need in our community.

    But if there are other possible ways to ensure that the number of psychologists continues to expand, I will also consider those possibilities as well.

    Okay. Sure. Thank you, chair and members of the committee. I've been on the contractors license board since 2021, and I have three years left to finish out my term. And I really enjoy, you know, helping to review contractors and make sure that the public are getting contractors that are legitimate contractors.

    And I'm currently chair of the board right now, and it's a challenge, but it's I think it's needed out there to keep contractors to the for the public to do work that are legitimate contractors, and our job is to review to make sure contractors that do get their license are legitimate contractors.     Thank you, mister Nishek. You know, as you can probably imagine, contracting is one of the, biggest areas, you know, for, people's complaints and concerns, particularly when people are looking for good contractors, etcetera. And in the past, there has been a big backlog, you know, with the contractors license board, applications pending, you know, before the board. As you have served since 2021 and seen, you know, various, different approaches being utilized.

    Do you have any recommendations for ways that, you know, we can kind of, eliminate the backlog, speed up the process, and, you know, ensure that we, move, review of the applications along as smoothly as possible.

    Yes. I think the biggest issue I think we have is a shortage of staff on the contractors license board executive committee. There had two people retire recently and so they're shorthanded and they're overloaded the staff that we have. I mean, we have, like, four applicant 400 applicants monthly. They're signed up to have their license reviewed, but just, you know, the shortage of manpower that also the board is actually shorthanded the members to be on the board.

    Right now, we have a minimum for quorum. So we're actually gonna be missing two meetings March and April because we don't have enough people on the board to have a quorum so we can have a full committee meeting. So any help in getting additional members on the board would be great and providing funding for more staff for the contractors association would be much appreciated.

    Well, we certainly would welcome that. And, you know, I know the department has been trying to upgrade its website, etcetera. Unfortunately, in the last few months, one of the things that happened was when they made that upgrade, then everything disappeared from the past. So we've received new complaints.

    So I hope that, you know, with your experience and your background, you and other members of the board will be able to help the CLB move forward as quickly as possible from the huge backlog that accumulated in the past. Thank you for your service.

    So coming up on 33 years now. Love my job. And I just want to make sure that everybody has a really good healthcare provider. So I'm honored to be considered for this nomination. I do stand on my resume and my testimony, and again I think this would be a wonderful opportunity for me to learn a bit more about this and also just to make sure that we continue to have really good health care providers, because we we deserve it here in Hawaii.

    Sure. You know I can certainly see the benefit of it, but we also need to maintain some sort of ability to make sure ourselves personally that the people that work here and take care of our patients here are qualified. So I I you know to be honest, I'm a little bit between the two categories right now.

    I want to make sure that we maintain some sort of ability to evaluate and to be a bit picky when it comes to the people that take care of the people here in Hawaii.

    I appreciate the response. I'm still unclear. Basically like, layman's terms, if we're sitting here as lawmakers next session, or if everybody's sitting here and they're discussing nursing compacts or any specific field, that would allow professionals from other states to come in here and maybe potentially solve that gap, but at potentially the expense of the nurses who are striking or whoever it may be that we always see year after year. Would I see your name in support and testimony in those, discussions?

    You know, that's a a tricky situation. Aye, you know, worked with the the nurses and other care providers here on Kauai for so long that, you know, I take care of their children. I see them at the store. I I truly respect their service to the community. I also want to make sure that my sick patient that needs, you know, urgent care can get that taken care of should something come up.

    So I know that's kind of a waffling answer. I apologize for that, But ultimately we want to make sure that everybody gets taken care of with their medical needs. But to be honest, I truly support all the caretakers, especially here on Kauai. You know, we've really we're always looking for physicians, nurses, respiratory therapists, physical therapists, etc. That want to work on the neighbor islands.

    And we want to make that as an attractive opportunity as possible, and protect those positions when they come here. Because I want them to to be able to enjoy their work, but also to thrive here in the community. So I'm definitely in support of that. And ultimately though, you know, it comes down if my little patient needs to be admitted to the ER, and I want to make sure that there's somebody there to take care of them in an urgent or emergent situation.

    Yeah. And and from from my side, I would like that person to be a somebody from here.

    Absolutely. I totally agree with you. Absolutely. You know, again, I want them to be somebody that's from here, that is somebody in the community, right? That's that's contributing to our community and gets to enjoy the the privilege of living here on the island of Kauai or any of the other islands.

    Doctor, I have a follow-up on that. So, you know, to Senator Awa's line of questioning, we have seen continually every year measures proposing to create, licensing compacts, particularly in healthcare for, counselors and therapists, for nurses, for psychologists, for psychiatrists, and for doctors. The the doctors compact was a priority of the governor's administration and passed into law, I believe, two years ago, Strongly advocated for by the administration and by the health care facilities.

    And I'm wondering, you know, whether on Kauai or in your experience in the healthcare setting, whether you have seen any kind of noticeable impact in access to care as a result of that compact or in the last two years?

  • Robert Wotring

    To be honest, I've seen not a lot of change. I do believe that we need more access to certain specialties, especially on the neighbor islands. We certainly have great specialists on the Island Of Oahu, and and thank goodness for telemedicine. That's made a huge difference. And and I think we're kind of leaders in The US with the use of telemedicine to kind of share the support of our really smart and talented people throughout the island.

    But sometimes that's not enough. So yeah, sometimes you know we need something in the interim until we can build up that kind of group of people that are really have the time and ability to take care of the people here.

    Okay. Well, even for telemedicine, those doctors still need to be located in Hawaii. Right?

    Sure. Yeah. Absolutely. Yeah. Or licensed here in yeah.

    So, just a quick follow-up. I'm curious whether because, you know, you are on Kauai. Correct?

    Person

    Correct.

  • Jarrett Keohokalole

    Yeah. So we don't see and I don't assume to understand what's happening on other islands where, you know, I'm not there every day. The year after the doctors compact was passed into law, the legislature and then this committee elected with the nurses to take a different tact, which was rather than approving a licensing compact, instead, we authorized the DCCA, licensing division to to, provide temporary licenses in lieu of full licensure because there is a tremendous backlog. Well, there was at the time.

    And as an interim measure to allow practitioners who are seeking to be licensed to obtain a temporary license so that they could start operating right away, rather than initiating this compact.

    And I'm wondering if from your experience on Kauai that's made a difference in the nursing context.

    I don't see a lot of difference, to be perfectly honest. Yeah. I wonder if it's maybe had a bigger effect on some of the other islands, but not as much here. Although, to be honest, I'm not definitely in on the the the minutiae of staffing in certain areas, so I may not know about that.

And now I'm on the board that actually takes care of that, and it's very interesting to see.
